placing a note directly on a reference link breaks the link

if i place an annotation directly on a reference link, selecting eg. "[8]", the link is broken.

not sure what happens if no endpoints are on the square bracket.

if no endpoints are on a square bracket (the link is inside the note) there are no problems.

i'm pretty sure this will happen anytime the start point of a note is within (or on the start of) an <a> tag.

we should probably throw an error in this case, and tell the user that they need a better start point - otherwise we'd need to close the existing <a>, add the note-bug <a> tag, and then reopen the existing <a>, which seems like a PITA.

Here is another example: In article http://ploscompbiol.org/doi/pcbi.1000162 the reader highlighted http://www.capalbo.nl and made a note, and now that link is inactive.
