	Affiliation (restricted list University/College, Research
Institute, Government Laboratory, Hospital/Medical Center, Commercial
Company, Private Individual)=20
	Institution (can this be mandatory conditional on the response
to "Affiliation". If Affiliation is "Private Individual" then
Institution is optional. Otherwise this field is optional)
	City
	State
	Country
	Display Name (as regards displaying this I would like it to be
the last field on the form and to be automatically filled with First
Name, Last Name unless overwritten by the user)
